Performance Anxiety Treatment in Mississippi (Situational Beta-Blocker Therapy)
Mississippi adult care by secure video visit, self pay option starting at $49, MD-only, insurance is not required.
Performance anxiety causes physical symptoms such as rapid heartbeat, tremor, sweating, shaky voice, and nausea before public speaking, presentations, auditions, interviews, or exams. These symptoms are driven by adrenaline acting on beta-adrenergic receptors and can be effectively managed with propranolol, a non-selective beta-blocker used off-label for intermittent situational anxiety. Propranolol is not a controlled substance, is not sedating, and does not cause cognitive impairment. It reduces the physical cascade of performance anxiety without affecting mental sharpness. TeleDirectMD uses a safety-first telehealth approach that screens for asthma, COPD, bradycardia, heart block, and other contraindications before determining whether propranolol is appropriate. If your symptoms suggest generalized anxiety disorder, panic disorder, or pervasive social anxiety rather than situational performance anxiety, TeleDirectMD will direct you to psychiatry or primary care for appropriate treatment. This page is for adults located in Mississippi, including Jackson, Gulfport, Southaven, Hattiesburg, Biloxi, Meridian, Tupelo, Olive Branch, Greenville, Horn Lake, and surrounding areas.

What Is Performance Anxiety?
Performance anxiety is a form of situational anxiety in which physical symptoms such as rapid heartbeat, tremor, sweating, shaky voice, dry mouth, and nausea occur before or during high-pressure situations like public speaking, presentations, musical performances, auditions, interviews, or exams. These physical symptoms are driven by adrenaline and norepinephrine acting on beta-adrenergic receptors throughout the body.
Performance anxiety is distinct from generalized anxiety disorder (GAD), panic disorder, and pervasive social anxiety disorder. In performance anxiety, symptoms are tied to specific triggering events and are not present daily. The physical symptoms can be disproportionate to the perceived threat and can significantly impair professional and personal functioning even in individuals who are otherwise confident and well-prepared.
Propranolol, a non-selective beta-blocker, is the established off-label treatment for the physical symptoms of situational performance anxiety. It blocks adrenaline at beta receptors, reducing rapid heartbeat, tremor, and sweating without causing sedation or cognitive impairment. Propranolol is not a controlled substance and has no dependency or addiction risk. TeleDirectMD evaluates adults for propranolol appropriateness by video visit, with careful screening for asthma, COPD, cardiac contraindications, and chronic anxiety disorders that need different treatment.
Causes and Risk Factors
Performance anxiety results from an exaggerated fight-or-flight response triggered by perceived high-stakes situations. The sympathetic nervous system releases adrenaline and norepinephrine, which act on beta-adrenergic receptors to cause the physical symptoms that characterize the condition.
- High-stakes situations: public speaking, presentations, musical performances, auditions, job interviews, and standardized exams are the most common triggers
- Prior negative experiences: a history of embarrassment, poor performance, or visible anxiety during past events can reinforce the anxiety cycle
- Perfectionism and high self-expectations: individuals who set extremely high standards may be more susceptible to performance anxiety
- Lack of experience or preparation: unfamiliarity with the performance setting can amplify the adrenaline response
- Underlying anxiety traits: some individuals have a baseline tendency toward heightened sympathetic nervous system reactivity, though this does not necessarily mean they have generalized anxiety disorder
Not every person with performance anxiety needs medication. Behavioral strategies, preparation, and cognitive-behavioral techniques are effective for many people. Propranolol is most useful when physical symptoms such as tremor, tachycardia, or shaky voice are the primary barrier to performance, and it works best in combination with adequate preparation and practice.
Symptoms and Red Flags for Performance Anxiety in Mississippi
Use this table to understand which symptoms fit situational performance anxiety and which suggest a need for in-person evaluation or psychiatric referral.
| Symptom or situation | What it suggests | Telehealth appropriate? | Red flag requiring in-person or psychiatric care |
|---|---|---|---|
| Rapid heartbeat, tremor, sweating before a specific event | Situational performance anxiety with physical symptoms | Often yes | If chest pain, fainting, or severe shortness of breath |
| Shaky voice, dry mouth, nausea before public speaking | Adrenaline-driven physical anxiety response | Often yes | If vomiting is persistent or associated with other medical symptoms |
| Anxiety limited to specific performance situations | Situational pattern consistent with performance anxiety | Often yes | If anxiety is daily, pervasive, or present in most social situations |
| Prior successful use of propranolol for similar events | Known responder seeking refill or continued access | Often yes | If new symptoms have developed such as wheezing, dizziness, or bradycardia |
| Daily anxiety, chronic worry, or difficulty functioning | Generalized anxiety disorder, not situational performance anxiety | Refer to psychiatry or primary care | Chronic anxiety needs SSRI/SNRI and possibly therapy, not intermittent propranolol |
| Panic attacks with sudden onset of terror and physical symptoms | Panic disorder requiring different treatment approach | Refer to psychiatry | Panic disorder needs structured treatment, not as-needed beta-blockers alone |
| Active suicidal ideation, self-harm, or substance abuse | Psychiatric emergency or complex mental health condition | No | Immediate crisis care: call 988 or go to the ER |
Differential Diagnosis: Performance Anxiety vs Other Conditions
Situational performance anxiety must be distinguished from chronic anxiety disorders that require different treatment. Propranolol for intermittent use is appropriate for isolated performance situations but is not a substitute for comprehensive anxiety management when a broader disorder is present. TeleDirectMD screens for these distinctions during every evaluation.
Sometimes Appropriate for Telehealth
- Situational performance anxiety with physical symptoms tied to specific events
- First-time evaluation for propranolol in adults without contraindications
- Refill of propranolol for known responders with stable situational use
- Guidance on trial dosing and timing before an upcoming event
- Discussion of behavioral strategies alongside medication
Often Requires In-Person or Specialist Evaluation
- Generalized anxiety disorder with daily symptoms needing SSRI/SNRI therapy
- Panic disorder with recurrent panic attacks
- Pervasive social anxiety disorder affecting most social interactions
- Known cardiac conditions including bradycardia, heart block, or heart failure
- Asthma or COPD requiring pulmonary evaluation before beta-blocker use
Performance Anxiety vs Generalized Anxiety Disorder
Performance anxiety is situational, tied to specific events, and primarily characterized by physical symptoms like tremor and tachycardia. Generalized anxiety disorder involves chronic, daily worry across multiple domains of life, difficulty controlling worry, muscle tension, and sleep disturbance. GAD requires SSRI or SNRI therapy and often cognitive-behavioral therapy, not intermittent propranolol.
Performance Anxiety vs Panic Disorder
Performance anxiety is predictable and tied to anticipated events. Panic attacks in panic disorder are often sudden, unexpected, and involve intense fear of dying or losing control. Panic disorder requires structured treatment with SSRIs, therapy, or both. Intermittent propranolol alone is not appropriate for panic disorder.
Performance Anxiety vs Social Anxiety Disorder
Situational performance anxiety is limited to specific high-pressure events. Social anxiety disorder is pervasive, affecting most social interactions, and involves persistent fear of embarrassment or negative evaluation across many settings. Social anxiety disorder typically requires SSRI therapy and cognitive-behavioral therapy, though propranolol may be used as an adjunct for specific situational triggers within a broader treatment plan.
If your symptoms suggest generalized anxiety, panic disorder, or pervasive social anxiety rather than isolated performance anxiety, TeleDirectMD will direct you to psychiatry or primary care for appropriate evaluation and treatment.

Treatment Options
Situational performance anxiety is most effectively managed with a combination of behavioral strategies and, when appropriate, intermittent propranolol for physical symptom control. Propranolol blocks adrenaline at beta-adrenergic receptors, reducing the physical cascade of anxiety without sedation or cognitive impairment. It is used off-label for this indication and is not a controlled substance.
Propranolol for situational use
Propranolol 10 to 40 mg taken approximately 1 hour before the anxiety-provoking event is the established approach for physical symptom control. It reduces rapid heartbeat, tremor, sweating, and shaky voice. It does not cause sedation and does not impair cognition. A trial dose should be taken before a low-stakes situation to assess individual response and tolerability before relying on it for an important event.
Behavioral and preparation strategies
Adequate preparation, practice in simulated environments, deep breathing techniques, and progressive muscle relaxation can all reduce performance anxiety. These strategies work well alongside propranolol and are recommended regardless of whether medication is prescribed.
Cognitive-behavioral therapy
For patients with a significant cognitive component to their anxiety, such as catastrophic thinking or excessive self-criticism, cognitive-behavioral therapy (CBT) can address the thought patterns that amplify the anxiety response. CBT is particularly useful when anxiety persists despite medication and behavioral strategies.
What propranolol does not treat
Propranolol reduces physical symptoms only. It does not address cognitive or psychological anxiety. A patient taking propranolol may still feel nervous but will not experience the visible physical symptoms that impair performance. If the primary problem is intrusive worry, rumination, or avoidance rather than physical symptoms, a different treatment approach is needed.
When to refer to psychiatry
If symptoms are daily, pervasive, or consistent with generalized anxiety disorder, panic disorder, or social anxiety disorder, TeleDirectMD will recommend evaluation by a psychiatrist or primary care provider for SSRI/SNRI therapy and structured treatment. Intermittent propranolol is not appropriate as the sole treatment for these conditions.
What TeleDirectMD Does Not Manage
- Generalized anxiety disorder requiring daily SSRI or SNRI therapy
- Panic disorder with recurrent panic attacks
- Pervasive social anxiety disorder needing comprehensive treatment
- Active suicidal ideation, self-harm, or psychiatric emergencies
- Performance anxiety in patients with asthma, COPD, bradycardia, or heart block
Common Medication Options
Propranolol is the primary medication evaluated for situational performance anxiety via telehealth. The actual medication, dose, and suitability are determined by the MD after reviewing your symptoms, medical history, current medications, and contraindications.
| Medication | Typical dose | Timing and duration | Key considerations |
|---|---|---|---|
| Propranolol (immediate-release) | 10 to 20 mg for first-time users | Take 1 hour before the event; effects last 3 to 6 hours | Start with a low dose for the first trial. Advise trial dose before a low-stakes situation. Not a controlled substance. No dependency or addiction risk. |
| Propranolol (immediate-release) | 20 to 40 mg for experienced users | Take 1 hour before the event; effects last 3 to 6 hours | Higher doses for patients who have tolerated lower doses but need more symptom control. Maximum single dose typically 40 mg for situational use. |
| Propranolol (long-acting) | 60 to 80 mg extended-release | Take 2 to 3 hours before the event; longer duration | Considered when the performance situation extends over several hours. Less commonly used for single short events. |
| Behavioral strategies (non-pharmacologic) | Not applicable | Ongoing practice | Deep breathing, progressive muscle relaxation, visualization, and rehearsal in simulated settings. Recommended alongside or instead of medication. |
| SSRI/SNRI (not prescribed by TeleDirectMD for this indication) | Varies by medication | Daily, ongoing | First-line for generalized anxiety disorder, panic disorder, and pervasive social anxiety. Requires ongoing management. TeleDirectMD refers to psychiatry or primary care for these medications when chronic anxiety is identified. |
Important: Propranolol doses listed are examples for situational use. The actual dose is determined by the MD after reviewing your medical history, current medications, and contraindications. Propranolol is not a controlled substance. TeleDirectMD does not prescribe controlled substances.
Home Care, Trial Dosing, and Follow-up
Trial Dose and Preparation
- Take your first dose of propranolol before a low-stakes situation, not before an important event, to assess your individual response
- Note the time from ingestion to effect (typically 30 to 60 minutes) and how long symptom relief lasts
- Monitor for dizziness, lightheadedness, or unusual fatigue during the trial
- Continue preparation and practice strategies for the upcoming event alongside medication
- Do not combine propranolol with alcohol, as both lower blood pressure and heart rate
What to Watch For After Taking Propranolol
- Dizziness, lightheadedness, or feeling faint may indicate the dose is too high or blood pressure is too low
- Wheezing, shortness of breath, or chest tightness are signs of bronchospasm and require immediate medical attention (stop the medication and seek care)
- Excessive fatigue or very slow heart rate may indicate sensitivity to beta-blockade
- If symptoms of anxiety are not adequately controlled, discuss dose adjustment at your next visit rather than increasing the dose on your own
Follow-up and Long-term Considerations
- Propranolol for situational use does not require routine monitoring in otherwise healthy adults
- If you begin using propranolol frequently (more than once or twice a week), discuss with your provider whether a different treatment approach is needed
- Consider cognitive-behavioral therapy if anxiety persists despite adequate physical symptom control
- If your anxiety pattern changes from situational to daily or pervasive, seek evaluation for generalized anxiety disorder
- Do not abruptly stop propranolol if you have been taking it regularly, as rebound tachycardia can occur

Performance Anxiety Treatment FAQs for Mississippi
Can I get propranolol for performance anxiety online in Mississippi?
Yes, if you are an adult 18+ located in Mississippi and your symptoms are consistent with situational performance anxiety after contraindication screening. TeleDirectMD can prescribe propranolol for intermittent situational use when clinically appropriate. Propranolol is not a controlled substance.
How much does an online performance anxiety visit cost in Mississippi?
TeleDirectMD offers a transparent self pay option starting at $49 for an adult video visit in Mississippi. Insurance is not required. Prescription costs at your pharmacy are separate. Generic propranolol is widely available and typically inexpensive.
Is propranolol a controlled substance?
No. Propranolol is not a controlled substance. It is a beta-blocker with no dependency or addiction risk. It is not sedating and does not cause cognitive impairment. TeleDirectMD does not prescribe controlled substances, and propranolol does not fall into that category.
How does propranolol work for performance anxiety?
Propranolol blocks adrenaline and norepinephrine at beta-adrenergic receptors throughout the body. This reduces the physical symptoms of anxiety including rapid heartbeat, tremor, sweating, and shaky voice. It acts peripherally, not centrally, so it does not cause sedation or impair mental sharpness. You may still feel nervous, but the visible physical symptoms that impair performance are reduced.
Should I try a test dose before an important event?
Yes. TeleDirectMD strongly recommends taking a trial dose before a low-stakes situation rather than relying on propranolol for the first time during an important event. This allows you to assess your individual response, confirm tolerability, and learn the timing of onset and duration of effect.
Can propranolol help with all types of anxiety?
No. Propranolol is effective for the physical symptoms of situational performance anxiety. It does not treat cognitive or psychological anxiety such as chronic worry, rumination, or pervasive social avoidance. If you have generalized anxiety disorder, panic disorder, or social anxiety disorder, you need a different treatment approach such as SSRI or SNRI therapy and possibly cognitive-behavioral therapy.
What are the main contraindications for propranolol?
Asthma and COPD are absolute contraindications because propranolol can cause bronchospasm through beta-2 blockade. Other contraindications include bradycardia, second- or third-degree heart block, decompensated heart failure, severe hypotension, and cardiogenic shock. Patients with diabetes should use caution because propranolol can mask hypoglycemia symptoms like tachycardia and tremor.
How long before an event should I take propranolol?
Immediate-release propranolol is typically taken approximately 1 hour before the anxiety-provoking event. Effects last 3 to 6 hours. Long-acting formulations should be taken 2 to 3 hours before. Your MD will recommend specific timing based on your situation.
Does Mississippi allow telemedicine for this kind of visit?
Yes. Mississippi allows licensed professionals to provide telemedicine within their scope when appropriate and according to accepted standards of care.
Can TeleDirectMD provide performance anxiety care in other states?
Yes. TeleDirectMD offers adult evaluations via video visits across multiple states. You must be physically located in the state where you are requesting care at the time of the visit.
What if I have asthma and need help with performance anxiety?
If you have asthma or COPD, propranolol is contraindicated because it can cause dangerous bronchospasm. TeleDirectMD will not prescribe propranolol in this situation. You should discuss alternative approaches with your primary care provider or a psychiatrist, as other treatment options exist for performance anxiety that do not involve beta-blockers.
Can I use my insurance for a TeleDirectMD visit?
Insurance is not required. If your plan is eligible, you may be able to use insurance. A self pay option is also available.
What if propranolol does not work well enough for me?
If physical symptoms are not adequately controlled, the dose may be adjusted at a follow-up visit. If the primary problem is cognitive anxiety rather than physical symptoms, propranolol alone may not be the right approach. TeleDirectMD may recommend cognitive-behavioral therapy, referral to psychiatry for SSRI evaluation, or a combination approach depending on your symptom pattern.
Is it safe to take propranolol with other medications?
Propranolol can interact with certain medications including other blood pressure medications, calcium channel blockers, and some antidepressants. The MD will review your current medication list during the video visit. Always disclose all medications and supplements you are taking.
